Please fill out all the fields including the contestant's name, age, and the date and location (city and state) of the contest you are paying for. In the "Total of Entry" section please include the total of all events and any membership fees (full membership payment or non-membership fees). Entry forms will still need to be sent to the contest director to have your twirler in the correct events for that contest. 

With all contests, please send twirlers entry form and submit payment before NOON of the deadline date.
